Passport fees and forms are slightly changing as of Feb 1, 2008. Anyone applying for a passport after Jan 31, 2008 must have the NEW, updated application. One change to passport services is an addition of a Passport Card. You now have an option to purchase only a Passport or both a Passport and Passport Card when applying. The new Passport Card is NOT valid for use to any destination by air. For more specific information and updates, please visit www.travel.state.gov .
The following items are required at the time of application:
* Certified copy of birth certificate must have embossed seal on certificate. Hospital birth certificates are not acceptable. You may also submit your most recent passport in place of the birth certificate if issued after the age of 16. Naturalized citizens must submit original naturalization certificate. All birth certificates, naturalization certificates, and old passports submitted will be sent with application and will be returned with the new passport.
* Two identical 2X2 photos. Photos must be in color and must have a white background. We do passport photos at our office for $15.00. There are also other locations who offer passport photos for various prices.
* Completed passport application. Blank applications are available at the State University Post office on campus currently at 104 S. Caraway Rd (our location will be moving in the student center hopefully by the first week of May). Or you can pick up an application at the Jonesboro Carrier Annex located at 2404 Race St or the Jonesboro Main Post Office at 310 East Street.
* Mail in renewal forms may be used for individuals who have had previous US Passport issued within the past 15 years and after their 16th birthday.
* Valid driver's license or state ID card. A valid US Passport may also be used for identification.
* Social security number is required on the passport application.
* Do not sign the application prior to appearing in person before executing official. The application must be signed in the presence of the acceptance clerk, who must witness your signature.
* If applying for a minor, the child must be present. For minors younger than 16 years of age, BOTH parents must be present in order to obtain a passport. For more information on minors, please contact us locally @ (870)932-7192 or visit www.travel.state.gov.
Fees are payable in the form of check or money orders. The fees will need to be separated into two different checks or money orders. One payment for all fees applicable to "The Department of State", and the other for all applicable fees payable to the "United States Postal Service". Postal money orders can be purchased at the time of application with cash or debit card. The fees shown below are the updated fees as of February 1, 2008. For the most current fees, please check with the local office or the travel website as shown above. The fees are as follows (with the exception of the postal money order fee):

Adult Passport: $ 75.00
Minor Passport: $ 60.00
Passport Card: $ 20.00
*Expedited Fee: $ 60.00
*Returned Express: $ 16.25
Payable to The United States Post Office:
Acceptance fee: $ 25.00
Photo services: $ 15.00
*Express mail: $ 16.25
*Expedited and express mail services are required when traveling within 6 weeks.
