Voluntary Veteran & Disability Disclosure Form

This form is available to new employees and existing employees for self-identification of veteran and disability status. This information is utilized to insure diverse recruitment and retention and non-discrimination efforts to serve and protect the needs of our university community. Disability status is not the same as qualifying for disability income or other government disability services. Here disability may include short or long-term circumstances, of a physical or mental health nature, for which an employee may require accommodation, now or in the future.
